Frailty Advanced Specialist Pharmacist (County Hospital )

Job summary

Are you a pharmacist looking for a new challenge, passionate about leading on the development of the clinical pharmacy service to the new Frailty Assessment Unity at County site? This is a novel and rare opportunity to introduce a new pharmacy service to an expanding front door field, alongside a pro-active multidisciplinary team.

This role is ideal for those who thrive in a dynamic, fast-paced environment, providing exceptional care to high acuity, complex patients. You'll be at the forefront of delivering a responsive, proactive, and person-centred clinical pharmacy service, working within your professional boundaries.

You will work alongside other Advanced Specialist Pharmacists and be part of the wider University Hospital of North Midlands Team, bringing unrivalled training, development, networking and practice research opportunities. Primarily you will work at our smaller (and very friendly) County Hospital site, situated next to the beautiful Staffordshire countryside.

You will contribute to the training and development of our dynamic team of junior pharmacists, as they complete their County Hospital rotations.

Main duties of the job

You will provide a daily clinical pharmacy service to new Frailty Assessment Unit at our Stafford (County Hospital) site, ensuring patients on time critical medicines are identified, prescribed and administered in a timely manner, providing safe and effective clinical pharmacy advice to medical and nursing colleagues, aiding trust flow.

Utilise the prescribing skill daily within your scope of practice (or work towards obtaining).

Review adverse incident reports and educate medical and nursing teams on pharmaceutical issues. Review and develop guidelines and PGDs.

Review expenditure reports and develop cost improvement initiatives.

Work with the Advanced Clinical Lead Pharmacist for Acute & Emergency Medicine on the strategic development of the clinical pharmacy service to County Emergency Department .

Contribute to the operational site leadership of County site.

Train, develop and line manage junior and trainee pharmacists rotating through County Hospital site.

  • Postgraduate Clinical Pharmacy Diploma / MSc or equivalent qualification

  • Commitment and evidence of continuous professional development (CPD)
  • Computer literate and able to use Windows applications including Microsoft Word, Excel, Teams and internet
  • Evidence of involvement in pharmacy practice research and/or audit
  • High standard of oral and written (including report writing) communication skills with attention to detail
  • Able to demonstrate good leadership skills
