Job summary
The trainee advanced clinical practitioner (ACP) endeavours to achieve through supervision, clinical and academic competence to become a qualified ACP. As a trainee ACP they will be an integrated member of the Cardiology team aiming to develop the capabilities to autonomously deliver a Cardiology service to the Emergency portals, Inpatient bed base and Outpatient Clinics across both UHNM sites- Stoke and County. Through physical assessment, diagnostic testing and interpretation, formulation of management plans will be made to ensure safe practice either as an Inpatient/Outpatient or referring to other specialties.

Practice as an exemplary role model throughout all four pillars of advance practiceAdvanced knowledge, skills and judgement for high level and complex decision making in an extended scope of practiceCompleting a full holistic health assessment using a range of assessment methods whilst working with patients, relatives and carersRequest imaging and investigations to underpin and support clinical decisions and diagnosisMake independent diagnostic decisions based on an expert clinical assessment and investigation interpretation when appropriateMake complex clinical decisions based on sound evidence base practice and clinical expertiseInitiate, rationalise and suspend prescribed treatments and medication, therapies, lifestyle advice and careCarry out invasive and non-invasive diagnostic and therapeutic procedures where appropriateRefer to other health professionals and services as appropriateDischarge patients from the service as requiredMaintain meticulous evidence of clinical specific competencies of advanced skillsExercise risk management using professional judgementCollaboratively work throughout all necessary multi-disciplinary membersProvide clear, clinical leadership across the whole clinical area, trust and partnerships when necessary
