Neonatal Community Outreach Nurse

Job summary

This is an exciting opportunity to join the well established UHNM Neonatal Community Outreach Team and be part of the Nursing Team responsible for supporting the transition from hospital to home for babies and families who have been cared for on the Neonatal Unit.

You will be utilising your nursing skills from hospital to home, and supporting babies with simple or complex health needs, including feeding support, home oxygen and home phototherapy.

Main duties of the job

To assess, develop, implement and evaluate nursing care needs of babies and families following discharge from the Neonatal Intensive Care Unit at UHNM into a community setting.

To participate and contribute in the delivery of a high quality Neonatal Community Outreach service.

To participate and contribute in teaching requirements of parents.

To rotate into the Transitional Care Unit/Neonatal Intensive Care Unit, to meet the demands of the service and fulfil professional educational needs.
