Job summary

Discharge Facilitator - Frail Elderly Assessment Unit (FEAU) - Ward 210An exciting opportunity has arisen for enthusiastic, highly motivated individual to join our team on FEAU as a discharge facilitator at Royal Stoke University Hospital.

FEAU is a 29 bedded medical emergency portal specialising in acute care of the older adult.

Main duties of the job

You will assist the MDT to plan and facilitate timely discharge processes for patients from point of admission to the unit environment to the discharge of patients to their own home or alternative care facilities or pathways.The post holder will have excellent communication skills and have the ability, drive and enthusiasm to work within a motivated team to provide high quality patient care the post holder will ideally have experience as a Discharge facilitator or relevant skills
