Job summary

An exciting opportunity has arisen for a driven, enthusiastic and highly-experienced Capacity Site Manager to join the Emergency Division management team.

The post holder will be responsible for the timely and appropriate placement of patients requiring acute or elective admission to hospital, on a daily basis, by proactive management and leadership.

Be responsible for maintaining an up to date overview of actual and predicted bed capacity and demand.

Provide up to date site management reports on bed base and demand and agreed outlined action plans, ensuring the Trust meet local and government targets.

Main duties of the job

The post holder will be responsible for the timely and appropriate placement of patients requiring acute or elective admission to hospital, on a daily basis, by proactive management and leadership.

Be responsible for maintaining an up to date overview of actual and predicted bed capacity and demand.

The post holder will possess the knowledge, specialist skills and experience to identify all triggers that could actually or potentially impact on patient pathways and implement action plans to assist in the resolution of such pressures. Ensuring patient safety and experience is paramount.

The post holder will possess specialist skills to effectively lead, support and co-ordinate an effective service; to be able to initiate prioritisation and delegation of workload and resources.

The post holder will be responsible for providing specialist advice and support to the wards and multi-professional teams in managing effective patient discharges an integral part of care management across the Trust.

Responsible for maintaining excellent internal and external channels of communication between staff, patients, relatives and foster an open and proactive working environment.

The post holder will be responsible for actively leading capacity and demand patterns ensuring the patient experience is at the centre of all decision making.

To lead bed and clinical site management, maintaining patient flow throughout the organization 24/7. Monitor staffing levels, risk assessing and at times where necessary moving staff appropriately to support clinical skill needs on the wards.

The post holder will be required to work proactively and under pressure, whilst maintaining professionalism at all times. Excellent communication skills are paramount to the success of this role, as the post holder will play a key part in maintaining operational resilience.

Liaise closely with all members of multidisciplinary teams to ensure accurate up to date knowledge of patient placement and flow is achieved.

Act as senior support when decision making in relation to the placement of patients requiring emergency and elective admission adhering to the Trusts infection control policy.

Liaise with other Trusts to appropriately repatriate patients from other hospitals back to their local authority.

Deliver accurate information of the Trust status at the Trusts operational bed meetings, on all specialties, ensuring that electronic data capture of demand and capacity, against the predictor and action logs are clear, with evaluation of progress on agreed actions.

Lead the establishment of systems to ensure the site teams consistent compliance with DoH SITREP reporting and monitor accordingly.

Lead and support in quality and improvement initiatives on flow and discharge through the hospital.

Escalating as per Trust policy potential and actual obstructions in patient flow and discharges and agree actions to problem solve with appropriate stakeholders.

Ensure patients are placed in the right place in a timely manner to support adequate flow and reduce outliers. Map, as appropriate, to their specialist wards when capacity available to minimise increased length of stay.

As required, support in the safe opening of escalation areas as per policy.

Work with other stakeholders, leading where appropriate to support admission avoidance strategies; support identified patients to Same Day Emergency Care. Oversea patient movement from the Emergency Department to assessment areas to achieve the 4hr access targets whilst maintaining patient safety at all times.

Foster and demonstrate creativity, liaising with the ward teams towards efficient patient flow and pathways.

Establish and maintain effective communication with individuals and groups about difficult or complex matters overcoming any barriers in communication.
