An exciting opportunity has arisen for an enthusiastic Band 3 Community Support Worker to come and join us in our Community Response Team. Our Community Response Team covers South Warwickshire, has bases at Stratford upon Avon and Leamington Spa working across the county supporting patients that have identifiedhealth and socialneedsas a resultofa change in their healthcondition.The serviceis committed toprovidehigh quality care toprovidean immediate response, aiming to preventavoidablehospital admissionsbyprovidingsupportto patients to remain independent within their own homeandfacilitatingsafe discharges home from hospital.The team workstogether to support patients in the community and work closely with social services, GPs, community nursing teams, patients,relativesand carers.
To be successful in this role you must hold a valid driving licence and have access to your own car.
The Community Response Team operates across the hours of 08:30-21:30, 7 days a week, including weekends and bank holidays. Shifts are allocated pro rata, we offer a mix of early, late and long days.
Main duties of the job
The Band 3 Clinical Support Worker will undertake high quality, caring and compassionate personal care and associated duties under the direction but not direct supervision of the registered staff and working as part of the multidisciplinary team.
As a Band 3 Clinical Support Worker (CSW) you make a valuable and important contribution to the delivery of high quality healthcare, supporting the registered professional in the delivery of safe, effective, compassionate patient care.
