Job summary
Urgent Response is a community based service. We are looking to recruit 2 motivated, dynamic and flexible Band 6 Senior Nurses within this service. This is a secondment /fixed term post for 12 months. We provide unplanned and urgent care as part of a multi-disciplinary community based team. The main base is in Nuneaton however, as the patients will have a Warwickshire GP, the work covers North Warwickshire. The role involves working as a member of the Urgent Response / Community Response team and responding to visits via Clinical Triage.
The role involves working in a leadership role as a member of the Urgent Response Team and responding to urgent and unplanned visits which originate via Clinical Triage. You will be assessing, planning, coordinating, delivering and evaluating the programmes of care provided across health, social and voluntary care sectors as part of a multi-disciplinary community based rehabilitation team to both facilitate and prevent hospital admissions. You will work within the community, clients home, residential and nursing homes and other sites as the service requires. The role involves being part of a multi-disciplinary team and having a holistic patient centred approach.
As a minimum you will be an experienced Band 5 nurse who wants to progress to a band 6 role and receive development support to assist you in achieving this aspiration.
