An exciting opportunity has arisen for an enthusiastic Band 3 Community Support Worker to come and join us in our Community Response Team. We have two teamssituated in Stratford upon Avon and Leamington Spaworking across the countysupporting patients that haveidentifiedhealth and socialneedsas a resultofa change in their healthcondition.The serviceis committed toprovidehigh quality care toprovidean immediate response, aiming to preventavoidablehospital admissionsbyprovidingsupportto patients to remain independent within their own homeandfacilitatingsafe discharges home from hospital.The team workstogether to support patients in the community and work closely with social services, GPs, community nursing teams, patients,relativesand carers.
To be successful in this role you must hold a valid driving licence and have access to your own car.
The Community Response Team operates across the hours of 08:30-21:30, 7 days a week, including weekends and bank holidays. Shifts are allocated pro rata, we offer a mix of early, late and long days.
Main duties of the job
Clinical Support Workers are members of the Multi disciplinary Team, who undertakes professional duties in the Patient's homes to ensure provision of high standard care, assisting with all aspects of daily living and promoting independence.
The Band 3 Clinical Support Workers will undertake high quality, caring and compassionate personal care and associated duties under the direction and supervision of the registered staff.
As a Band 3 Clinical Support Worker (CSW) you will make a valuable and important contribution to patients' quality of life, supporting them to remain safe at home.
Furthermore, there are opportunities for further training and career education progression.
