We are looking for an innovative and highly motivated Community Nursery Nurse to join our integrated team of Health Visitors, Community Staff Nurses, Nursery Nurses in our Southam Health Visiting team, with a genuine interest in the health and wellbeing of children and their families in South Warwickshire.

We provide a universal service to all families and children within the 0-19 age group, targeted in response to the identified needs of the population and are committed to taking forward the local Children and Young Peoples Plan.
You will be committed to working in partnership with other key agencies and supporting Health Visitors leading developments in the local community. We are looking for people with energy and enthusiasm, as well as a high level of professional skills, knowledge of the current health agenda and a drive to change.
NNEB/BTEC National Diploma/NVQ 3 in Early Years or equivalent is essential for this post, as are excellent communication skills and the ability to Prioritise and organise workload.
Main duties of the job
The CNN will work as a member of the Health Visiting team in the delivery of health education/health promotion and the Healthy Child Programme. The CNN will assist the Health Visitor in the planning and delivery of local public health initiatives.
The service works in close collaboration with its local authority and voluntary and community sector partners in order to ensure that health priorities are embedded across the range of provision to achieve improved outcomes for children and young people in line with Every Child Matters: Change for Children, Choosing Health, The National Service Framework for Children, Young People and Maternity Services.

The Community Nursery Nurse (CNN) is responsible for assisting in the identification and assessment of the health needs of both children and families.
To undertake or participate in universal developmental reviews, including heights and weights of pre-school children as delegated by the Health Visitor.
To work closely with the Health Visitors in the planning and delivery of health promotion activities and public health initiatives in a variety of settings.
To carry out home visits as a lone worker when delegated by a Health visitor.
To carry out delegated family support interventions or programmes of planned care in partnership with children and carers and other agencies under the supervision of the Health Visitor. This can include delivery of health promoting activities in homes, health care or community settings, with individuals and groups.
To contribute to the assessment of health needs and delivery of appropriate services to meet needs of children, families and the wider community.
To provide and promote access to information and other services, to refer as appropriate.
To liaise and work in partnership with statutory and non-statutory agencies, and other health services colleagues.
To work in partnership with local Childrens Centres and other agencies to encourage and involve parents with programmes of support. To input in the planning and delivery of those services.
To contribute to the Common Assessment Framework, where delegated by the Health Visitor, and to contribute to Early Support and act as Lead professional where appropriate.
To utilise skills of observation and recognise any significant change in client circumstances and report back to the Health Visitor.
To work with the Team Leaders to support children, young people and families who are considered to be vulnerable or at risk. To be aware of and adhere to current approved policies and procedures of Warwickshire Safeguarding Board.
To undertake mandatory training, and service specific training as required maintaining skills and competencies.
To participate in regular 1:1 clinical supervision, appraisals and clinical rounds with delegated Health Visitor and attend team meetings.
To support and contribute to the development of integrated team working by modelling effective team practice and good communication skills.
To be an advocate of service change and future development.
To ensure that individuals within the teams are aware of and work within the Trust policies procedures.
To develop and maintain effective and appropriate communication with staff and colleagues within the Health visiting teams, Childrens centres and Early Years Settings.
To take reasonable steps to see that the working environment is safe, healthy and hazard free for staff and users, this includes ensuring that infection control procedures are in place.
To be competent in the use of information technology and ensure that relevant and up to date information on clinical and service developments are provided to agreed time-scales.
To participate in audit, research projects and/or innovations following agreed policy and protocol.
To participate in the induction of newly appointed team members.
To maintain accurate, legible and contemporaneous records, deal with highly sensitive information, respecting confidentiality and security at all times in accordance with trust policies for data protection.
All incidents (and near misses) must be reported to the appropriate health visitor within the team and then recorded on the Datix System.
