Job summary
South Warwickshire NHS Foundation Trust is looking for a caring, compassionate, highly self-motivated, honest, trustworthy, reliable & enthusiastic Clinical Rehab Support Worker to work on Campion Ward based at Leamington Spa Hospital. Campion is a 22-25 bedded unit supported by GP. We care for patients who require sub-acute admission prevention, dementia care & rehabilitation or palliative care.
This new and exciting position is suitable for anyone looking to 'develop' their career as a Rehabilitation support worker. You will be part of the multi-disciplinary team and have the ability to work flexibly/autonomously and efficiently with good organisational skills under the guidance of qualified therapists that enable you to work on your own initiative contributing to the smooth running of a very busy department.
Your role will be to support and assist healthcare professionals such as nurses, doctors and other integrated professionals in the delivery of quality patient care and the effective rehabilitation of our patients. You also must have excellent communication and interpersonal skills, be reliable and work well in a team.
At interview you will be expected to demonstrate knowledge of the importance of rehabilitation of our patients although you can expect further training and if successful will have competencies to meet going forward.
Main duties of the job
Working as a Clinical Support Worker, the post holder will be expected to apply a rehabilitation approach to all aspects of their role; promoting functionally independence in all activities of daily living.
The post holder will undertake roles and duties delegated by a suitably qualified and regulated professional, including nursing and therapy staff.
They will work collaboratively with the team to meet the needs and co-morbidities of patients; following structured treatment plans for rehabilitation.
The post holder will continuously and autonomously review and assess patient's level of functional ability.
They will liaise with appropriate professionals to support the ongoing rehabilitation needs of the patient, progressing and adapting treatment plans under guidance.
The post holder will follow practice policy and procedures and will champion standards of rehabilitation and care across the service.
