Job summary
Clinical Integration and Development Managers willprovidehighly visible clinical leadership across their geographical area of responsibility.They will be the local clinical 'face of SWAST' at tactical and strategic level meetings with commissioners,providersand wider partner organisations,representingthe Clinical Directorate and the Paramedic profession.
Reporting to the Head of System Integration, the post holder will lead definedlocal and Trust wideworkstreams that translate strategic priorities intopractice, withmeasurable improvement. The role will work collaboratively with the wider clinical team, operations, and corporateservices, as well as external system partners, to improveclinical care,referral pathways, care coordination, and overallhealthsystem performance. They will support the implementation of national strategies, including the NHS 10 Year Health Plan,annual Urgent and Emergency Care Plans,the TrustsStrategyand annual plans.
The post holder will provide senior clinical leadership, working with operations teams to enable the delivery of the right care, in the right place, at the right time. The role will focus on improving patient flow, strengthening urgent and emergency care pathways, and maximising the proportion of patients who can be safely managed without conveyance to an Emergency Department.
Main duties of the job
Lead assigned system integration workstreams in line with Trust strategy and priorities.
Work with commissioners and providers todevelop,supportandoptimisethe availability and capacity of Single Points of Access (SPoA), alternativecommunityand hospital pathways.
Support delivery of clinical referral optimisation, including hearand treat(with support of EOC colleagues), see and treat, and community pathway utilisation.
Be a key member of the Integrated County Clinical Team (ICCT).
Work with the wider ICCTto reduce variance inkey clinical metrics includingsee and treat rates and on-scene times between operational areas.
Support the implementation and development of Care Coordination Hubs and system processes for Category 2-5 performance.
Develop and maintaineffective working relationships with ICS partners, community providers, and system operational leads.
Develop clinical guidelines, SOPs and associated documents which ensure that patients receive care which reflects the latest evidence base.
Lead on new and revised clinical interventions being implemented into practice.
Monitor, analyse, and report on system referral activity, performance, and outcomes,identifyingtrends and opportunities for improvement.
Contribute to the development of system-wide service improvement and transformation initiatives.
The post holder will be expected toparticipatein an on-call rota relevant to their knowledge, skills, and experience.
