Job summary

Be an expert in the Patient Safety Incident Response Framework (PSIRF) learning responses, investigations, techniques and practice. Manage members of our Quality team - focusing on the Emergency Operations Centre elements.

Undertake formal Patient Safety Incident Investigations (PSIIs), After-Action Review (AARs), Coroners reviews, using PSIRF methodology, producing high quality investigation reports and presenting findings to various audiences both internal and external.

Provide expert advice and support to appointed Investigating Officers in the investigation of patient safety learning responses and serious complaints, drawing on experience.

Prepare and deliver training to staff and managers on investigation skills, including PSIRF methodology and associated processes.

Main duties of the job

Be the identified Quality Lead responsible for managing PSIRF learning responses for the Trust and undertake the formal investigation of Patient Safety Incident Investigations (PSIIs), After Action Reviews (AARs) and Coroners reviews relating to the activities of the Trust's Emergency Operations Centre (EOC) in accordance with Trust policy, best practice and national guidance.

Produce high quality patient safety and experience investigation reports ensuring that, where appropriate, system issues are identified and recommendations made for consideration and discussion.

Undertake incident reviews for inquests and clinical negligence claims involving the Trust, producing a high quality report to be provided and/or presented to Coroners court, Trust solicitors and other interested parties.

Contribute to learning responses, round table reviews and other patient safety investigations and reviews undertaken by external organisations, ensuring they accurately reflect the Trust's involvement.

Provide specialist expert patient safety incident investigation advice and guidance commensurate with knowledge and specialist training provided to patient safety team investigators by those organisations identified by NHS England.

Line manage members of the Quality team (EOC), ensuring they meet the objectives of the Trust.
