Job summary
We are excited to be developing adult mental health rehabilitation services across Cornwall as part of the NHSE transformation programme. Our service includes a 12-bedded inpatient rehabilitation unit alongside a growing Community Mental Health Rehabilitation Team, working together to support smooth transitions and help people live well and independently in their communities.
We support people with complex psychosis who may benefit from short inpatient admissions for symptom stabilisation, medication review, physical health monitoring and support with everyday living skills. As our service continues to grow, we have created a dedicated transition pathway and are offering a fantastic opportunity for a compassionate and motivated Registered Mental Health Nurse to lead this work and support our Social Inclusion Workers.
In this role, you will take a genuinely person-centred, recovery-focused approach, using the Dialogue+ model to co-produce meaningful, individualised care plans. You will play a key role in supporting timely, safe and sustainable discharges from acute inpatient wards, helping people move forward with confidence. Strong communication and relationship-building skills are essential, as you will work closely with service users, families and carers, acute services, crisis teams, HTT, housing providers, VCSE partners and patient-flow teams. You will also provide clinical supervision and support to Social Inclusion Workers, each managing a short-term caseload.
Main duties of the job
To be responsible for the assessment of care needs and clinical management of the rehabilitation outreach caseload within the Cornwall Partnership NHS Foundation Trust (CFT) Mental Health Service.
To carry continuing responsibility for the development, implementation, and evaluation of programs of care, utilising the Lead HPC/ co-ordination system.
The Social inclusion team lead is expected to provide clear clinical operational leadership to the Community Team including the social inclusion team, staff nurses and healthcare assistants. They will do this, first and foremost by themselves providing exemplary practice.
This post requires you to be office based the majority of the time
They will also take on specific management duties within the Community Team that are commensurate with their grade and provide general management in the absence of the Community Manager.
They will provide clear clinical leadership and direction in the on-going benchmarking and good practice service evolution.
To develop the interface between primary and secondary care services so the patients/carers are offered focused and coordinated packages of care that are appropriate to their needs.
