We have a vacancy to join our team to continue strengthening community support for adults with Learning Disabilities, in the east community team in Cornwall.
This full-time Band 6 or Band 5-6 developmental Community Nurse post joins an established nursing and MDT team in West Cornwall. You will undertake nursing duties aligned to agreed Care Processes and Pathways, working closely with individuals, families and carers to deliver person-centred assessment, intervention, monitoring and review. You'll provide evidence-based practice within an innovative multidisciplinary team supported by strong supervision and appraisal. The team and wider trust offer CPD, research and audit opportunities, with excellent service-user engagement and joint working across Acute and Primary Care liaison, Social Care, Mental Health, Forensic and Children's services.
Clinical work includes multidisciplinary assessment of complex cases, formulation and delivery of therapeutic interventions, planning and supervising work for HCAs, and providing patient-centred training packages to care providers.
Main duties of the job
As part of the Community Learning Disability Team you will provide person-centred, skilled and effective assessment, intervention, monitoring and review for designated individuals who are eligible for the Learning Disability Service. You will be responsible for the maintenance of evidence-based learning disability nursing practice, contributing to and facilitating Care Processes and Pathways. You will manage and supervise the work of others, and act as a mentor and preceptor.
