Job summary
We are looking for a Senior Nurse, experienced working with patients in the community with complex long-term conditions.
Covering as a secondment or fixed term contract you will join with our North Kerrier Community Matrons/Long-term Conditions Nurses who, as part of the wider Community Nursing Team,work closely across Adult Community Services, Adult Social Care and Primary Care, using enhanced clinical skills to enable patients to remain independent at home for as long as possible and with the aim of reducing the risk of avoidable hospital admissions.
We are looking for someone who has completed the HEAB 358 Advancing Practice in Long Term Conditions Management module or the equivalent. The post holder should be an independent prescriber or working towards this qualification
The post holder will need a high level of communication, problem-solving and decision-making skills.
This is an exciting opportunity as you will be expected to work in partnership with others to develop, take forward and evaluate direction, policies and strategies as we seek to expand our proactive frailty approach within the North Kerrier Neighbourhood Team.

Main duties of the job
Autonomous Clinical Practice:
Ability to take a comprehensive history, perform a systematic physical examination and establish baseline data to inform the development of a comprehensive and personalised care plan.
Integrate care across all care settings; preventing duplication, fragmentation and delay occurring as patients move between care settings
Establish the dynamic process of understanding, reflecting, active listening and checking understanding, thus developing a therapeutic relationship with patients and their carers.
Education:
Provide the knowledge, and utilise coaching to empower patients and families to make informed decisions and participate in care planning.
Play an active role in disseminating knowledge about chronic disease and its management within the Community Nursing Team, mentoring students as required and developing education for other colleagues.
Leadership/Management:
Contribute to the development of policies, procedures and guidelines, ensuring they are implemented and monitored.
Research:
Develop at least one special area of expertise to be seen as a local expert, able to articulate the most contemporary evidence, and approaches to practice and management.
