Job summary
Exciting opportunity to play a pivotal role in enabling safe and timely discharge, through active discharge planning from the point of admission to reduce delayed transfers of care.
Effective discharge planning provides a multidisciplinary approach to continuity of care; a process that includes identification, assessment, goal setting, planning, implementation, coordination, and evaluation. Effective discharge planning supports the continuity of care, providing a critical link between treatment received in hospital by the patient, and post-discharge care provided in the community.
The post holder will be a health or social care professional with expertise of working within Mental Health services.
This post sits within the Patient Flow team, this incorporates bed management and discharge coordination.The wards are spread over two hospital sites, Bodmin Hospital and Longreach House.

Main duties of the job
- Ensuring that the dignity and respect of service users is maintained.
- Signpost service users to gain access to resources including benefits and welfare rights
- Follow planned interventions and monitor service users progress. Using apragmatic and practical approach to working with people with mentalhealth problems.
- Preparing agreed assessment reports/treatment plans accurately in the accepted format.
- Making applications for funding discharge packages based on MDT recommendations.
- To maintain adequate records as required by existing procedures, entering appropriate details on service users case notes as necessary, show awillingness to attend training to facilitate recording information electronically.
