An exciting opportunity has arisen for an experienced, caring, motivated support worker to join our small team of Primary Care Liaison Nurses. The team is focused on delivering high standards of care ensuring equal access to all primary care and eligible screening services for people with a learning disability.
Ability to travel independently across the County to fulfil the requirements of the post is essential.
Main duties of the job
As a Senior Support Worker, you will be an active and positive member of the Countywide Primary care liaison team and screening liaison team for adults with a learning disability contributing skills and experience while working within the teams' protocols and pathways. To work with individuals with a learning disability, or those individuals whose names are on the GP surgeries learning disability registers their families and carers. To help identify patients whose names should be on the GP learning disability register using the inclusion tool (assessment) and accessing patient notes to determine if a patient has a learning disability diagnosis.
You will liaise with, and work alongside primary care staff to help identify and support with meeting the health care, and screening needs of people with a learning disability, supporting the use of reasonable adjustments to, in order to improve the health outcomes for those individuals.
