The NHS 10-Year Health Plan sets out clearly that neighbourhood health is at the centre of our future model for local communities and populations.
This role will help shape an appropriate model of delivery for our communities within Central Cornwall which keeps the patient at the centre of our work.
This critical post will support the place director within Central ICA to develop and implement integrated services which provide seamless delivery of care for patients at the right place, at the right time and by the right individual. Central Cornwall has 6 Integrated Neighbourhood Teams (INTs), all of whom have delivery plans set out by their steering groups that we will be working on together to implement.
The ICA development manager will be instrumental in leading the development of these plans by working closely with the INTs.
Main duties of the job
This role is critical to help us develop our system wide approach to delivering neighbourhood health. The post holder will be responsible to convene and influence system partners to share a common purpose and responsibility to meet the four core purposes of integrated care systems (ICS). These are to improve outcomes in population health and healthcare; to tackle inequalities in outcomes, experience and access; to enhance productivity and value for money and to help the National Health Service (NHS) support broader social and economic development.
