We are looking to recruit an enthusiastic, Patient Flow Co-ordinator, within the Cornwall Onward Care Team, based in Carew House, Bodmin. This person requires previous experience in patient flow.

An exciting opportunity to support patient flow from an acute hospital working alongside an experienced team.

The aim is to achieve a collaborative approach to hospital discharge and is to ensure that patients only stay in hospital for the time required to complete their acute treatment, and that, where necessary, they experience timely and seamless transfer of care to another setting.

To assist with the efficient day to day running of the Patient Flow Team, providing administrative and clerical support to Nursing, Cornwall Council staff and any personal in Carew, supporting the Onward Care Team.

Main duties of the job

  • To ensure patient's needs are addressed based on the Care plan and coordinated in the most timely and effective way.
  • To act as a link, liaising with other agencies and departments, to provide efficient and effective communication to expedite progress towards there onward care.
  • Develop working relationships with ward teams to enable them to support and co-ordinate flow across the acute trust.
  • In collaboration with clinical teams, health and social care agencies, Patient Flow Co-ordinator will support the Integrated Transfer of Care Hub in planning and managing the daily flow through the Acute trusts.
  • Will contribute to follow up assessments, Care planning, identification of resources and service delivery in order to facilitate the appropriate and timely discharge of patients.
