Job summary
The Community Rehabilitation and Home First teams are part of the Penwith Integrated Community Team. They include registered and non-registered nursing, therapy and administrative staff, working with partner organisations to support the intermediate and rehabilitation needs of the local population.
This position is open to those who wish to develop assessment, care planning, reablement and rehabilitation skills whilst being part of a dynamic, research-active, progressive, integrated, multiagency team.
We offer a supportive working environment in which to develop your clinical and interpersonal skills, a structured in-service training programme; appraisal and supervision system.
We are keen to hear from anyone considering full or part time hours. This is a community-based role that requires frequent travel to various locations throughout the county including areas that may not be accessible by public transportation. Successful candidates will need to demonstrate how they will manage these travel requirements.

Main duties of the job
You'll be responsible for assessment, planning and implementation of care, and assist in the interpretation of information relevant to the patient's condition/progress, identify problems and needs and set realistic rehabilitation goals. You need excellent organisational skills.
You will need to be able to provide specialist evidence based treatment plans for adults within the catchment area, either in their own homes or care homes.
You will work alongside the wider multidisciplinary teams within the community which includes partner agencies, liaising with the community and acute hospitals, Adult Social Care and the Voluntary Services
This post covers the Penwith are which spans from Hayle to Marazion and all areas to the West.
The service covers a 7 day rota which involves some weekend and bank holiday working: the successful candidates must be flexible with their working days. The service operates between the hours of 8am-6pm
