We are creating a morepatient-centred, community-based approachthat brings compassionate care closer to home in the last year of their life or nearing the end of life.

We have opportunities available across various localities within the county, offering flexibility in base location depending on service need and candidate preference with a requirement to cover Cornwall and the Isles of Scillys.
Main duties of the job
To assist in the provision of nursing care to patients in the community, regularly working withoutdirect supervision but as a member of the proactive nursing team.To assist in the completion of patient and routine records, report information to trained staff onprogress of patients and any variance/change in patient/carers care management.To participate in any relevant training and updating programmes and keep abreast of nursingdevelopments, to improve the quality of care for patients.To report to manager/trained nurse any untoward incidences (eg complaints, accidents, defectiveequipment) and take appropriate action to ensure safety of patients, staff and relatives.To comply with Trust policies and procedures in order to contribute to safe working practices inaccordance with health and safety policies.
