Job summary
Cornwall Partnership NHS Foundation Trust is looking for a dedicated and experienced Speciality doctor to join a dynamic and friendly team with ambitions to achieve excellence within the home treatment team based at Bodmin Community hospital, Bodmin, Cornwall.
This role will be supported by a Consultant Psychiatrist, with opportunities for the right individual to enhance their skills and develop though the pathway route to specialist registration or as a multi professional approved clinician.
Main duties of the job
A key part of this role will be weekly case reviews with the Home Treatment Team as well as liaising with Community Mental Health Team Consultants to ensure continuity of care, good communication and joined up care. The post holder will work with the Home Treatment Team manager to continue to improve the service and review relevant performance data. They will assess new referrals to the Home Treatment Team and review case notes of patients that are not accepted into Home Treatment Team. They will contribute to the review of existing patients gathering history and examining mental states. They will also prioritise patients' physical health issues carrying out physical examinations and liaising with primary care and other health agencies where appropriate to ensure quality and consistent care in line with local and national guidance.
The post holder will attend weekly multidisciplinary team meetings where they will be asked to keep medical notes and where their clinical contribution will be welcomed and encouraged.
As time and experience allows, there will be the opportunity to prepare tribunal reports and occasionally other medical reports and to attend related hearings.
There are no designated outpatient clinics; however it may be appropriate to develop clinics as an efficient way to review community patients. It is expected as this post is community based that home visits will be a significant part of clinical activity. The post holder will not carry a personal case load.
