You'll provide clinical pharmacy services to Community Hospitals across Cornwall. Your base location is negotiable, though travel to other sites will be required. Working patterns, including a rolling weekend rota, are flexible and open to discussion for the right candidate.
We aim to recruit at Band 5, but we are open to a Band 4 to 5 development opportunity for suitable applicants.
This varied role will require working independently and collaboratively within the wider pharmacy team. As a Medicines Optimisation Pharmacy Technician, you'll support medicines reconciliation, prescription reviews, discharge facilitation, ward-based dispensing, medication recycling, patient counselling, and audits across both physical and mental health inpatient teams.

Main duties of the job
To provide a comprehensive medicines management service to patients on these wards which includes medicines reconciliation, prescription review and endorsement, recycling of medication, supporting self-medication and, where necessary, patient and carer counselling.
To participate in multi-disciplinary activities such as attending ward handover meetings and facilitation and preparation of discharge medication including the handover of information regarding a patient's medication to other healthcare professionals if required.
To work with the pharmacy team in delivering clinical pharmacy and safe systems of working with medicines across other community hospital and mental health sites.
To supervise and support with education and training activities such as student technicians, student pharmacists and pre-registration pharmacists
To specialise if the opportunity arises as per service needs and personal interest (for example, Parkinson's, Medicines Safety)This post will support hospitals within Cornwall. Base location can be negotiated but there will be a requirement to travel to other peripheral hospitals.
