Job summary

This is an exciting and autonomous role supporting the design and development of a community based urgent care response as part of the Ageing Well Programme which will align with the Embrace Community Intervention Offer (CIO). This is a role requiring an innovative and tenacious clinician, with a proven track record or recognised potential for embracing and leading change. This role will require vision and enthusiasm to help codesign and transform our evolving community services within Cornwall. There is a need to test new models of care delivery and advanced practice roles, this will necessitate a flexible and responsive individual who is confident to adapt and work with autonomy and ambiguity.

Main duties of the job

The enhanced clinical level of this role will require you to be able to undertake enhanced level comprehensive assessment including history taking, physical examination of the patient and a robust systems review. You should be able to assess and manage frailty and complex chronic disease, identify deterioration and implement immediate strategies to reduce the risk of acute admission. Furthermore you will need to be able to recognise the early symptoms of disease exacerbation, acute illness and injuries, based on an understanding of chronic diseases, the disease process, current evidence and practice standards.

You will be expected to be a practitioner who can demonstrate a broad range of competencies who is able to adapt to a wide variety of clinical circumstances. You will be part of a community team where your proven ability to make high level clinical decisions will aid patient recovery. This will include direct working with Home First, and community nursing teams, in addition collaboration with GPs, Cornwall Council, voluntary partners, secondary care, and community hospitals. This role is pivotal in supporting the implementation of the Ageing Well programme and the Embrace Community Intervention offer (CIO).
