Job summary

This vacancy is open to applicants currently employed by Cornwall Partnership NHS Foundation Trust, Royal Cornwall Hospitals NHS Trust, or the Integrated Care Board for Cornwall and the Isles of Scilly.

Cornwall Partnership NHS Foundation Trust is shaping a new vision for Community and Palliative End of Life Care across the county. Our goal is to support people nearing the end of life to have a calm, peaceful, and dignified death in the place of their choice.

Cornwall currently has one of the highest rates in the country for hospital stays during the last 90 days of life. This highlights the need for a patient-centred, community-based service that allows more people to receive the care they need at home.

We're building a service that reduces unnecessary hospital admissions, offers a single point of access, and delivers personalised care plans for home-based support.

In partnership with the Macmillan Social Investment Fund and local organisations, we're expanding services, including a 24/7 Rapid Community Response and Care Service, and a dedicated phone line for both the public and professionals focused on End of Life Care.

These developments will complement our existing Hospital@Home virtual service and new partnerships with the voluntary sector to enhance local care.

Main duties of the job

This autonomous role involves working with commissioners, GPs, partner organisations, secondary care, community teams and community hospitals to support the development of new and existing services regarding the clinical skills required to manage our patients. Some will have complex care needs being treated closer to their homes or in the immediate community setting.

The post holder is the clinical specialist lead for clinical skills facilitation. They will develop training programmes to support and empower staff in the care of patients including those with highly complex needs in the community setting.

Ensure appropriate education is developed, delivered, and evaluated in relation to a wide range of clinical skills. This education will be delivered in the classroom and clinical settings working alongside clinical teams to ensure appropriate access to education.

They will have line management responsibilities for the clinical skills team and be responsible for several policies relating to clinical skills.
