Job summary
This vacancy is open to applicants currently employed by Cornwall Partnership NHS Foundation Trust, Royal Cornwall Hospitals NHS Trust, or the Integrated Care Board for Cornwall and the Isles of Scilly.
Cornwall Partnership NHS Foundation Trust is shaping a new vision for Community and Palliative End of Life Care across the county. Our goal is to support people nearing the end of life to have a calm, peaceful, and dignified death in the place of their choice.
Cornwall currently has one of the highest rates in the country for hospital stays during the last 90 days of life. This highlights the need for a patient-centred, community-based service that allows more people to receive the care they need at home.
We're building a service that reduces unnecessary hospital admissions, offers a single point of access, and delivers personalised care plans for home-based support.
In partnership with the Macmillan Social Investment Fund and local organisations, we're expanding services, including a 24/7 Rapid Community Response and Care Service, and a dedicated phone line for both the public and professionals focused on End of Life Care.
These developments will complement our existing Hospital@Home virtual service and new partnerships with the voluntary sector to enhance local care.

Main duties of the job
To provide responsive and expert clinical care for patients with palliative and end if life care (PEOLC) needs as part of palliative care services.
The purpose of the post is to lead in the development of virtual palliative and end of life care for people with palliative and end of life care needs, including people with both malignant and non-malignant conditions and people in the last year of life with multiple chronic diseases.

Approximately 568,000 people live here. A third of people who live in Cornwall are supported by acute hospital services in Devon. As a result, we also work closely with our partners in Devon. In the summer, and during other holidays lots of people choose to visit the area. This increase the numbers of people who use out services.
