Job summary

An exciting opportunity has arisen for a Band 5 Occupational Therapist post in the Carrick Home First Team providing assessment, reablement and rehabilitation to adults with health problems. This includes creating and reviewing care plans, reablement plans to facilitate independence and self-management and rehabilitation plans for support workers to implement. The teams support the Ageing Well Programme with 2 Hour Urgent Care Responses, 48 Hour Reablement Responses and Anticipatory Care Responses. The team plays a key role in supporting prevention of admission to Bedded Care and Discharge to Assess pathways.

We are seeking innovativepre and postregistration Clinicians who will relish this creative approach and career development within the wonderful county of Cornwall.

Structured supervision and opportunities to participate in the established in - service training programmes and support for continued professional development will be provided

This is a full time post working 37.5 hours per week.

Main duties of the job

Responsible for the assessment, planning, implementation and evaluation of care, without direct supervision, to patients, carers and relatives within their home environment, enabling them to enjoy maximum independence and optimum quality of life.
