Job summary

We are a multi disciplinary community team which provide secondary health services to adults with a learning disability.

Full time and part time hours will be considered.

Please note this is a countywide community post and the successful applicant will be required to travel within the geographical area without relying on public transport.

Main duties of the job

As a Senior Support Worker you will be an active and positive member of the Community Learning Disability Team, contributing skills and experience to the clinical assessments and interventions. Working with the individuals with a learning disability and their families you will monitor, contribute to, and modify the Care Plan in response to need, in order to improve the health outcomes for the person.

Approximately 568,000 people live here. A third of people who live in Cornwall are supported by acute hospital services in Devon. As a result, we also work closely with our partners in Devon. In the summer, and during other holidays lots of people choose to visit the area. This increase the numbers of people who use out services.

Essential

  • NVQ Level 3 in Health and Social care or equivalent relevant experience/qualification
  • Previous experience of working with people with a learning disability with complex needs, some of whom exhibit behaviour that challenges, in a variety of settings.

Desirable

  • Experience in a variety of settings. Experience working with people with complex behavioural and/or mental health needs

Skills and Aptitude

  • Excellent communication and record keeping skills. Ability to recognise and report with accuracy the people with a learning disability needs and/or environment.
