Job summary

We are seeking to recruit an experienced community support worker who has worked with people experiencing psychosis and other mental health conditions.

The successful candidate will be working in the new "At risk mental state" (ARMS) team within the Early Intervention for Psychosis Service. (EIPS).

This role will include supporting individuals to improve their mental health by working with them, their families, members of the multidisciplinary team and wider support agencies within the community.

This vacancy will be covering Mid Cornwall working inline with the area model of care.

Main duties of the job

  • Establish and maintain effect contact with service users, their relatives and carers in order toensure good communication.
  • Give information and assistance to service users and their carers so that they are aware of theirrights, entitlements from the team and wider health and social care community.
  • Work with service users and their carers to access identified services.
  • Monitor mental health and be aware of fluctuations in mental health through observationsand report changes to the team practitioners so that service users are directed to appropriateservices and risk is managed.
  • Encourage social/physical activities as appropriate to the service user's needs
