Job summary

This is a community-based role that requires frequent travel to various locations throughout the county including areas that may not be accessible by public transportation. Successful candidates will need to demonstrate how they will manage these travel requirements.

We have an exciting opportunity available for an enthusiastic and highly motivated individual to join the Carrick Community Mental Health Team.

Our multidisciplinary team strives to deliver high quality and effective services to people experiencing severe and enduring mental health problems. We offer assessment and treatment of mental health problems, with a focus on effectiveness and recovery, working to promote the independence of our service users.

The team are looking for someone who can demonstrate the Trust's vision and values in practice, are motivated to be involved in innovation and service development and and are passionate about mental health.

As an ICMHT team member you must have both a positive outlook and sense of humour to accept the challenges of working in a busy locality that extends to seaside towns and rural locations.

You will need to have knowledge of general mental health problems, the impact this may have on service users and their treatment.

You will receive regular support and supervision from the Management and Leadership team.

Main duties of the job

  • To be responsible for the assessment of care needs and clinical management of a defined caseload within the Cornwall Partnership NHS Foundation Trust (CFT) Mental Health Service.
  • To carry continuing responsibility for the development, implementation and evaluation of programs of care, utilising the care co-ordination system.
  • To develop the interface between primary and secondary care services so the patients/carers are offered focused and co-ordinated packages of care that are appropriate to their needs
  • You will need to have knowledge of the CPA process, legislation around mental health and The Mental Health Act. You will need excellent communication skills and place value on team and inter-agency working. We would like to work with an individual who wants to utilise their skills to effect positive changes for our service users.
  • To demonstrate passion and commitment to mental health issues, have an excellent approach to client-centred care along with working flexibly as part of a team, to deliver innovative health and social care.
