Job summary

This is an exciting opportunity to be involved in a new programme of work, that utilises technology to enable individuals to remain in their own home as an alternative to bedded care. Virtual Ward provision is instrumental in enabling people to receive hospital level care in their normal place of residence, the benefits of which are very significant to the individual and the wider health system.

The Virtual ward utilises remote monitoring in conjunction with completing face to face visits to enable people to stay in their homes

The service works closely with Royal Cornwall Hospital, Community Matrons, UCR, Specialist Nurses from Cardiac and Respiratory services, Practice Nurses and GPs with prevention and early detection of deterioration at the forefront of our intervention.

Main duties of the job

The Virtual Ward team triage referrals, admit patients including a holistic assessment and in conjunction with medical colleagues create a care and monitoring plan for the patient. The patients symptoms and clinical observations are then monitored and care reviewed as required.

This role includes both face to face patient care as well as remote monitoring.
