Job summary

This is an exciting opportunity for an experienced mental health practitioner to join the Homeless Vulnerability Liaison Service (HVLS) a county wide provision. The trauma informed service was commissioned by Cornwall council rough sleeper initiative in 2019 and we continue to develop an expand. The team is a multi-agency service working in conjunction with Pentreath, Rethink, Cornwall Mind and adult social care.

Our client group have multiple needs to include: mental illness mild to severe and enduring, Trauma based need, substance misuse, developmental disorders, acquired brain injury and LD. We work closely with Health for Homeless GP practise, rough sleeper services, accommodation providers to name but a few.

The service is part of the Centre for mental health and justice, based at Bodmin police hub, with a large proportion of our work being carried out in mid and west Cornwall. The successful candidate will be subject to police vetting and require a full driving license.

Main duties of the job

Provide assessment of mental health wellbeing and psychological vulnerabilities to the client group to assist in moving toward safe and appropriate accommodation

Work in partnership with housing , criminal justice and voluntary sector organisations to facilitate engagement with services to address complex needs around accommodation

We work with those who are rough sleeping, sofa surfing, in emergency or temporary accommodation and supported accommodation, who present with vulnerabilities relating to their mental health and psychological wellbeing. Our aim is to support clients to improve their wellbeing and increase the likelihood of sustained tenancy's.

HVLS is a brief intervention service to engage individuals with a view to referral and sustained engagement with mainstream services. We offer psychosocial assessment, psychology intervention, health and wellbeing recovery work and adult social care intervention, as well as having access to consultant psychiatrist time one day a month.

At present HVLS does not offer a crisis service or urgent assessments. Referrals are triaged by the team at the weekly MDT and allocated to the appropriate team member based on identified need.
