Job summary
An exciting opportunity has arisen for a full time vibrant Registered Mental Health Nurse to work in our Community Psychiatric Rehabilitation Service, which is based at the Bodmin Hospital site in Cornwall.
Fettle Community Outreach Rehabilitation team provides a service across the whole county of Cornwall supporting people recovering from complex and enduring mental health issues. The community Rehabilitation Team operates alongside the inpatient rehab ward at Fettle House allowing the team to follow through a service specific pathway for our patients as they resume/establish independent lives outside of the hospital setting.
We take a person-centred approach in developing individual care plans and goal setting. In addition, the post holder will have a key role supporting and supervising junior team members and in the absence of the manager taking on the management role as well as participating in the continued review and development of the rehabilitation service. Regular individual support and supervision is provided.

Main duties of the job
To be responsible for the assessment of care needs and clinical management of the rehabilitation outreach caseload within the Cornwall Partnership NHS Foundation Trust (CFT) Mental Health Service.
To carry continuing responsibility for the development, implementation, and evaluation of programs of care, utilising the care co-ordination system.The Senior Staff Nurse is expected to provide clear clinical operational leadership to the Community Team including staff nurses and healthcare assistants. They will do this, first and foremost by themselves providing exemplary practice.
The Community Rehabilitation team works as part of a wider team that includes carers, families, other specialists e.g., Forensic services, housing, education, pharmacy,
They will also take on specific management duties within the Community Team that are commensurate with their grade and provide general management in the absence of the Community Manager. Provide clear clinical leadership and direction in the on-going benchmarking and good practice service evolution.
To develop the interface between primary and secondary care services so the patients/carers are offered focused and co-ordinated packages of care that are appropriate to their needs.
This is a full-time post with the requirement to be flexible in working shifts across two shift patterns 07:55- 20:38 and 11:00- 23:00 including weekends.
