Job summary

An exciting opportunity has arisen for an enthusiastic, innovative, flexible and well-motivated Occupational Therapist to join the Community Rehabilitation Therapy Team based in Central Cornwall. This post will be based primarily in Carrick Community Rehabilitation Team based at Falmouth Hospital.

We deliver Occupational Therapy in Central Cornwall Locality to individuals with a Primary Physical Health condition. You will work as part of a team of Occupational Therapists, Physiotherapists, and Therapy Support Workers. Our work is primarily within the person's own home environment although it may be appropriate to work in other locations ie nursing/residential homes, day care facilities or GP surgeries. Our geography is large and driving is essential. Our intervention is targeted at a wide variety of conditions, eg long-term, neurological, orthopaedic, falls, frailty and usually multifactorial in presentation .

As part of integrated services we work along side and closely with our community nursing colleagues and home first.

We also have close links with our specialist therapy services and inpatient colleagues and joint work when appropriate.

Main duties of the job

  • To manage a defined caseload of patients with complex needs within the Carrick Community rehab team using evidence-based practice, Occupational Therapy process and patient-centred principles
  • To assess, plan, implement and evaluate interventions as an autonomous practitioner using independence of judgement
  • To provide leadership for Junior and support staff through supervision, training and appraisal

To participate in the planning and development of Occupational Therapy services within the Integrated Therapy Service.

It will be an expectation of this post that if needed the person works flexibly across all areas of the Central Integrated Therapy Team to include inpatients and home first. Seven-day working could become an operational requirement of this post in the future.

Currently the service is commissioned Mon - Fri excluding Bank Holidays.Regular clinical supervision will be provided and continuing professional development encouraged.
