Job summary
Exciting opportunity for enthusiastic and highly motivated individuals to join our Integrated Community Mental Health Team based in Truro.
Carrick ICMHT strives to deliver high quality and effective services to people experiencing severe and enduring mental health problems, working as an interdisciplinary team. We offer assessment and treatment of mental health problems, with a focus on effectiveness and recovery, working to promote the independence of our service users.
We are looking for people who have a core profession in mental health nursing, social work or occupational therapy to join our community service line. We want people who have experience or who are looking for an experience in working independently in a mental health setting. As an ICMHT team member you must have a positive outlook and sense of humour to accept the challenges of working in a busy locality that extends to seaside towns and rural locations. You will need to have knowledge of general mental health problems, the impact this may have on service users and their treatment. You will need to have knowledge of the CPA process, legislation around mental health and The Mental Health Act. You will need excellent communication skills and place value on team and inter-agency working. We need a person who can demonstrate passion and commitment to mental health issues have an approach to excellent client centred care along with working flexibly as part of a team, to deliver innovative health and social care
Main duties of the job
- To be responsible for the assessment of care needs and clinical management of a defined caseload within the Cornwall Partnership NHS Foundation Trust (CFT) Mental Health Service.
- To carry continuing responsibility for the development, implementation and evaluation of programs of care, utilising the care co-ordination system.
- To develop the interface between primary and secondary care services so the patients/carers are offered focused and co-ordinated packages of care that are appropriate to their needs.
