Job summary

The Stroke Specialist Nurse (SSN) will work across 7 days on a rotational basis, as a key member of the ICSS Team to ensure all patients in a defined area who have had a stroke, or TIA have access to specialist assessment and associated care.

Service Aims:

  • Improve management and assessment of risk factors for patients with stroke/TIA
  • Improvement of long-term care for patients with stroke/TIA management
  • Early identification and management of stroke related complications
  • Reduce acute admissions
  • Improving access to other services
  • Providing advice and signposting to counselling

The SSN will monitor progress, provide follow up and risk assessment, recognise new symptoms related to stroke (and refer to specialties as necessary) and continue to provide ongoing advice and support in relation to stroke, and TIA.

Main duties of the job

  • Manage a caseload of appropriate clients, planning and co-ordinating own and team day to day work load in collaboration with the needs of patients, carers and the wider service
  • Mentor students allocated to the team and teach on pre and post registration courses
  • Participate in community and inpatient based patient/carer/family education and support programme
  • To provide clinical support to the senior SSN"s within the Locality Team
  • To undertake interdisciplinary working alongside members of the ICSS team
  • To participate in TIA clinics or TIA Clinic as required
  • To provide follow up, either at home or in a clinic setting, for stroke and TIA patients as deemed appropriate by the Locality Team lead
  • To provide patient and carer education and support
  • To assess and deliver care and support/information/advice to stroke and TIA patients under the direction of the Locality Team lead
  • Participate in projects in support of the Countywide Team Lead
  • Support the development of service guidelines, policies and procedures
  • Maintain up to date client records in a timely and accurate manner using the IT system as required by the organisation
  • Complete own GP and referral letters in a timely manner using agreed letter templates
  • Enter clinical activity on the IT system as required by the organisation
  • Complete SSNAP data capture forms and send to the administration team ready for uploading in a timely manner
  • Accurate, complete and up to date entry of patient and activity data onto trust systems
