The inpatient unit is called Carn Brea/ CATU ( Community Assessment and Treatment Unit ) whichis a unit working towards a model of reducing frailty admissions to the Acute trust by providing acute medical , nursing and therapy interventions.

The other part of the contract will involve working in the North Kerrier community rehabilitation Team, visiting patients in their own homes including care homes and working on promoting independence and achieving their personalised goals.

Regular clinical supervision and training opportunities will be provided. You will experience shared clinical education of Physiotherapy students and be responsible for supervising support workers and more junior team members.

Main duties of the job

We are looking for a colleague who has experience of working in an inpatient and/or community setting. Your clinical duties will involve providing high quality Physiotherapy assessments and treatments to varied and complex caseload with multi-pathology / long term conditions and acute medical complications with a strong focus on multidisciplinary team working. You must have excellent communication and health coaching skills with an empathetic and caring nature.

You will need to have excellent organisational skills combined with experience of working in older person's services.

This post is based in Camborne Redruth Community Hospital and split between 3 days in the Community Rehab Team and 2 days on Community Assessment and Treatment Unit (CATU). As this post involves completing community and outreach visits in patients own homes, you will need to have a valid UK driving licence and access to a car to travel to different locations, at times outside the North Kerrier area to facilitate a discharge.

This is a very supportive environment, and would be an ideal post for progression from a Band 5 position, or for an experienced Band 6 to gain skills working in a different team.

You will be a part of 7-day service and will be required to provide weekend cover on rotational basis.

We work in people's homes, in community clinics and bases. Some staff work from one of our 13 community hospitals. Our aspiration is to have great people, provide great care, be a great place to work and a great partner.

Approximately 568,000 people live here. A third of people who live in Cornwall are supported by acute hospital services in Devon. As a result, we also work closely with our partners in Devon. In the summer, and during other holidays lots of people choose to visit the area. This increases the numbers of people who use our services.
