Job summary

This post is part of CFT's Centre for Mental health & Justice, a countywide service integrated within Cornwall Partnership NHS Trust's mental health services.

We are closely aligned to the low secure ward based in Bodmin and a priority area will be to support patients to move toward the least restrictive environment and community placements.

The post holder will need to deliver organized proactive Care coordination to an identified group of clients Cornwall is responsible for subject to Ministry of Justice restrictions and conditions.

We will now be looking to deliver specialist interventions and care coordination to an extended client group and deliver the 4 SWPC tiers model. This exciting development will now allow us to deliver time limited joint working including face to face intervention with ICMHTS and other teams.

The role is to provide a specialist forensically based case ordination and management, assessment, formulation and intervention service to clients of the Centre for Mental Health and Justice and to offer advice and consultation as part of an established MDT

The post holder will need to be wiling to manage a caseload both in and out of county and work flexibly across the county to meet the needs of the whole team client group

This post is subject to satisfactory Devon and Cornwall police vetting as we are co located in police sites.

Base: Bodmin Police Hub . County wide working including regular out of county travel

Main duties of the job

To provide care co-ordination and specialist community forensic team interventions of assessment, consultation, care planning and evaluation to a designated group who meet the criteria for service delivery within the Community Forensic Services.

To actively engage with other secure services pathways and other providers both internally to Cornwall and out of county to deliver high standard of care to the complex client group of mentally disordered offenders and or those with forensic needs

The post holder will also engage with the south west provider collaborative developments to provide time limited joint working with ICMHTS
