Job summary

Caradon Integrated Community Mental Health team is a progressive and recovery focused team with a strong emphasis on facilitating high quality, effective and evidence based interventions with people who experience severe and enduring mental illness. We are a team of highly motivated and supportive professionals who work to ensure we empower individuals to independently manage their own mental wellbeing.

We are looking for a Senior Mental Health Practitioner who is a confident and motivated practitioner with excellent communication skills who has a professional background in mental health nursing, occupational therapy or social work; you will demonstrate clinical practice that is consistent with the vision and values of Cornwall Partnership NHS Foundation Trust. You will have experience, or will be looking for experience, in working independently in a community mental health setting; you will be able to work autonomously and as part of a multidisciplinary team.

Main duties of the job

To be responsible for the assessment of care needs and clinical management of a defined caseload within the Cornwall Partnership NHS Foundation Trust (CFT) Mental Health Service.To carry continuing responsibility for the development, implementation and evaluation of programs of care, utilising the care co-ordination system.To develop the interface between primary and secondary care services so the patients/carers are offered focused and co-ordinated packages of care that are appropriate to their needs.

  • Current professional registration - NMC, HCPC
  • Professional qualification - RMN, OT, Social worker
  • Evidence of post basic training

Desirable

  • Teaching & Assessing in Clinical Practice
  • ENB A12 Advanced Assessment skills

  • Knowledge of current legislation, national policies and guidance relevant to people with a mental health diagnosis
