Job summary

The Dementia and Older Person's Service (DOPMH) in Cornwall Partnership NHS Foundation provides assessment and treatment to people living with dementia and adults with functional mental health difficulties and increased complexity associated with physical ill health and the physiology of ageing.

This post will be based with the Gannel DOPMH community team that forms an essential part of the pathway from early diagnosis to end of life care. The team is also supported by a memory assessment nurse, Primary care dementia Practitioners, a dementia liaison nurse, Health Care Assistant, a Team Manager and consultant.

We are looking for highly motivated individuals who are passionate about working in this field and who are looking to join an innovative team at the cutting edge of services delivering high quality safe and effective care that people who use it say is good .

You will be a qualified professional RMN and have experience of working with people with dementia and/or mental health difficulties and be able to manage a caseload under the care programme approach placing the person at the centre of care in a rewarding but busy role.

The ability to work in partnership with carers/family members and services in the community is essential. We will provide you with management and clinical supervision and the opportunity to develop your skills in this area of work through training and development.

This post will be based at Trevissome House.

It is full time substantive post.

Main duties of the job

The post holder will be part of a multi-disciplinary team, contributing to the effective provision of Mental Health Services to an identified caseload of service users.

The post holder will work as a senior member of the team and liaise with a wide range of both statutory and non-statutory agencies to provide creative packages of care.
