Job summary
We are looking for a practical and caring person, who is enthusiastic about rehabilitation, to join our Older People's Inpatient Rehab Team based at Queen Mary's Hospital, Roehampton.
Patients have a wide range of complex conditions associated with older age including falls, neurological conditions (such as Parkinson's Disease) post operative rehabilitation, dementia, frailty, fractures and other medical problems.
We provide continuing rehabilitation for patients transferred from local acute hospitals, before they are able to be safely discharged home. There may be the opportunity to see patients in their own homes for discharge visits, where indicated, alongside a physiotherapist or occupational therapist
Clinical experience of working as a therapy assistant or student in an NHS or caring environment is essential.
Queen Mary's Hospital is a purpose built Community hospital with access to a wide range of on site and community services. The ward has 42 beds and you will be part of the wider multi-professional ward team including Nurses, Health Care Assistants, Doctors, Dietician and Social Workers. There are also close working relationships with other Older People's community rehab services.
There will be a requirement to undertake some administrative duties to support the team, as part of the role
Main duties of the job
You will be working closely with the Occupational Therapists, Physiotherapists and Speech and Language Therapists carrying out exercise programmes, practicing personal care and domestic tasks with patients, group activities and home visits in preparation for discharge back into the community.
You will have an allocated caseload of patients and, in conjunction with the therapists manage them, progressing their rehabilitation as appropriate.
You will also frequently do joint sessions with the Physiotherapists, Occupational Therapists, Speech Therapists and other Rehabilitation Assistants, with lots of opportunities for learning and development
