Job summary
To assess patients admitted to St George's with heart failure following an appropriate referral or through actively monitoring for admissionsof patients with suspected heart failure. To provide evidence based specialistcardiology nursing care to all patients. To support innovation and ensure quality isembedded into daily practice.
Main duties of the job
To work as a specialist practitioner using advanced skills to effectively deliver an expert nursing service to patients with a diagnosis of heart failure.To be able to work both individually and as part of the multidisciplinary teamTo assess patients admitted to St George's with heart failure following anappropriate referral or through actively monitoring for admissions of patients with suspected heart failure.To manage a caseload of patients/clientsTo ensure that patients and their families are fully informed and supported byproviding clinical and social supportTo assist in the development of evidence based clinical nursing practice in line with current research and guidelinesTo establish clear lines of communication within the heart failure team to ensure cohesive multidisciplinary management of patientsTo link with healthcare teams in order to increase knowledge and facilitate thecare of this group of people in the acute and community settings.? To work with the team to develop the Heart Failure Unit philosophy and models ofcare to ensure that all policies and procedures are evidence based, implemented and embedded and to lead on the development of nursing practice.To assess the effectiveness of current practices within the Heart Failure Unit and work with the team to introduce innovations that lead to improved quality of care
