Job summary

We are looking for an Administrator in the Older People's Advice Liaison Team (OPAL) at St Georges Hospital. We are a friendly dynamic team working in the Emergency Department and the Acute Medical Unit.

The role requires excellent computer skills with strong communication skills, and may involve delicate conversations.

Main duties of the job

  • To contact patients next of kin (NOK) and collect an extensive collateral history to contribute to the Comprehensive Geriatric Assessment and document it in a clear and professional manner.
  • To use information gathered from the next of kin (NOK) to create a care plan in conjunction with the Multidisciplinary team ( MDT) to print it and laminate it so that it can be taken with the patient throughout the hospital to advise staff about the patient and how best to care for them
  • Contacting Relatives, Social Services and GP'S to gain information about patient's and their social function/medical history.
  • Completing referrals to community teams for the frailty practitioners.
  • Recording electronic data for the team.
  • To proactively deal with potential situations of conflict, to diffuse and prevent patient complaints.
