Base: St George's Hospital but the post holder may be required to work at any of the Trust's sites.

Accountable to: Matron

Reports to: Ward Manager

Responsible for: The admission and safe discharges of inpatients and supporting junior staff on the ward.

We have an opportunity for a Registered Nurse to join our friendly team as a Band 6 Admissions and Discharges Coordinator! Working hours for this role are 08:00 - 16:00 Monday - Friday.

Benjamin Weir Ward is a 28 bedded cardiac surgery ward. We care for patients pre and post cardiac surgery, as well as cardiology procedures.

This role requires excellent communication skills in order to liaise with key members of the multi-disciplinary team, as well as primary and secondary care and community services. You need to have the necessary skills to provide information to patients to ensure safe and timely discharges. Previous cardiac surgery or patient discharge experience would be an advantage but the right person will have the opportunity to develop their skills as part of a highly capable team where you're guaranteed opportunities to learn.

Main duties of the job

  • To ensure patient's admission & discharge care needs are assessed.
  • To provide leadership to facilitate excellent nursing care that promotes rehabilitation.
  • To ensure all nursing procedures are carried out in accordance with Trust policies.
  • Liaise with multidisciplinary team, be responsible for setting, monitoring and maintaining excellent standards of care.
  • Maintaining personal contact with patients, relatives and carers being sensitive to their needs.
  • Managing the ward if requested by the Ward Sister and co-ordinating the clinical team.
  • Ensuring that accurate and timely records of discharge planning assessments, planning, interventions and contacts are maintained.
  • Storing, checking and administering drugs in accordance with Trust policy. Co-ordinating the early preparation and safe delivery of TTO's.
  • To work in an unsupervised capacity throughout all areas of the speciality.
  • To prioritise own work load to facilitate a seamless and safe discharge.
  • Undertake assessment of all patients' admission and discharge needs in appointed area of responsibility without supervision.
  • Co-ordinate and refer to appropriate therapy teams.
  • Refer to and liaise with Social Services Departments and other local agencies as appropriate.
  • Co-ordinate the admission and reception of new patients.
  • Assessing and booking patient transport as needed.
