Transitional Care Sister/Charge Nurse

Job summary

  • The job holder will offer professional advice and support to women whose babies require NTC.
  • Identifies deteriorating babies and takes the appropriate action to manage the situation, liaising with the multidisciplinary team as appropriate.
  • Acts as a resource for advising the multidisciplinary in areas such as health promotion, parental anxiety, and general advice.
  • Works, in partnership with parents/carers to assess, teach, support, and guide them to carry out effective care of the infant requiring a higher level of supervision on the postnatal wards.
  • Safely administer prescribed medications and monitor their effects.
  • To maintain up-to-date procedures, policies, and guidelines applicable to the service and to write NTC reports as required.
  • Provide NTC with the aim to support mothers as the primary care providers for their baby's routine new-born care, including other aspects of care that do not require admission to the Neonatal unit but can be provided on TC.
  • To support initiatives towards achieving and sustaining NTC in line with the Trust's guidelines, striving to raise the NTC provided in line with the BAPM Framework for transitional Care.

Main duties of the job

  • The post holder will participate in the daily operational management and leadership of Transitional Care ensuring delivery of high quality care for babies and their families.
  • To participate as a member of the neonatal multi-disciplinary team providing evidence based family centred care.
  • The post holder is responsible for the assessment of care needs, the development, implementation and evaluation of programs of care without supervision - undertaking care in the most appropriate environment using a high level of technical expertise.
  • Skilled in communicating effectively with parents, families and members of the multidisciplinary team to facilitate continuity of care and effective team working.
  • To manage and lead the nursing team and other staff members of the multidisciplinary team in the clinical area ensuring high standards of clinical care is delivered.
  • To keep accurate and completed records of activities and communications consistent with legislation, policies and procedures.
  • Participate in the teaching and supervision of junior staff and students.
  • Actively promotes Educational development within the unit and service.
