Job summary
This post aims to improve the experience of people, living in Wandsworth and wider SW London boroughs with highly complex long term neurological disabilities, as they journey though the health and community care service system. The key aspects of the neuronavigation role are defined as 'providing direct support to the local acute and specialist rehab services, with expert clinical advice, assessment and education, and directing patient referrals to the most appropriate specialist service' (London Neuronavigator Service Operating Model).
Main duties of the job
- To in-reach proactively into local hospitals, specialist units and other services e.g. Wolfson (Queen Mary's Hospital and St Georges Hospital), RHND QEF and other inpatient units as required.
- To undertake in-depth assessments of service users with complex physical / neurological disabilities and apply the findings from these assessments to a complex care or neuronavigation plan.
- To proactively support complex patients in navigating the health and social care system.
- To ensure that patients are referred to the most appropriate service to meet their needs.
