Job summary

The Patient Pathway Co-ordinator is responsible for the operational delivery of their Specialtiesinpatient admissions service. Key tasks for the role are:o To ensure the smooth running of elective and trauma admissions.o To schedule theatre lists in liaison with patients, consultants, and capacity.o To ensure all appropriate pre-operative assessments have been carried out to allow theatre lists to run asplanned.o To manage and validate a large waiting list and to assist in ensuring all patients are operated on within themaximum waiting time targets.o To ensure trauma patient are booked in a timely manner in line with the COW's advice.o To ensure end-to-end booking process as agreed with patient - patients will informed with reasonable noticeand sent all relevant paperwork for admission dates and fasting instructions if applicable. These areaccurately recorded on the appropriate PAS system.o To ensure accurate and complete computer records are maintained for patients.o To action Past TCI report and ensure patients are admitted and discharged properlyo To contribute to service developments.o To train new admissions staff and ensuring high standards of service and compliance with the trust's values.

Main duties of the job

Main Duties/Key Results AreasAdmissions processo Add the patient to the Waitlist on iClip.o Identifying and booking patient from WL or those planned by clinicians.o Call the patient and confirm their date.o Refer patient to POA team if appropriate.o Complete end to end booking - iClip, transport, language line, text, email, shared spreadsheet etc.o Send confirmation letter with all required documentation e.g., fasting details, leaflets.o Arrange preadmission bed if required so.o To list plan with your consultant on weekly basis.o To liaise private patient team if required. To be responsible for ensuring that all patients are issued with asuitable date by the private secretary and to answer any enquiries generated by the change of venue.Cancellations and DNAso Investigate any patient DNAs and deferrals, communicating the results to the Consultants and Managers asappropriate, updating iClip, waitlists and databases accordingly.o Patient cancellations - communicate the details to the consultant ensuring that all information is available.Notify staff involved and update iClip, waitlists and databases accordingly.o Hospital cancellations - communicate with the patients to re-organise a suitable date and update iClip,waitlists and databases accordingly.

Job responsibilities

General Duties To be responsible for ensuring that each patients status is updated and checked regularly to ensure thattreatment is received at an appropriate time, by an appropriate surgeon in an appropriate place. To organise the immediate booking of the patient on to the list decided upon by the consultant, ensuring thatall relevant information is available and that iClip is updated accordingly. Patients marked urgent or soon are discussed with the consultant and given an appropriate dateimmediately which is then communicated to the patient by letter. To contact the patients to be cancelled as soon as possible and to deal with them in a sympathetic andsensitive manner, explaining the situation and the possible alternatives. To update iClip and local spreadsheet accordingly. To cross cover and take on tasks as delegated by the Assistant Service Manager and Service Manager.Waiting List Management To take responsibility for reviewing and validating the waiting lists on a regular basis and to ensure thatappropriate follow-up action is taken. If the patient cannot be contacted, they will be sent a contact letter (PB2). Inform your consultant if surgery no longer required. To ensure that routine patients are added to the appropriate waitlist. Bring any long waiters/urgent to the attention of the Consultants and the Manager to prevent waiting timebreaches. To report validation position to your ASM on weekly basis. Remove duplicate entries. Removing patients who no longer need/want surgery or who are incorrectly listed Check historic (Past TCI) Validate long waiters Validate priority status with clinicians Follow Access Policy guide.Communication with patient When routine patients are allocated a date, they will be contacted and offered a reasonable offer as suggestin the Access Policy document. Confirm with the patient that the offered date is convenient and if not, negotiating a new date taking intoconsideration the waiting times and booked theatre lists. To answer politely and promptly to telephone enquiries from all patients and professionals, and to offer theappropriate support to any caller as required.Managing People To train junior staff, ensuring that they receive appropriate training and can provide the required level ofservice.
