8A Clinical Team Leader: OT/ PT
We are looking for an experienced and enthusiastic Band 8a AHP leader who is responsive to patient and staff population needs and is committed to person centred care within domiciliary setting.
You will be overseeing the delivery of the following services in localities across Wandsworth borough, incl St George's and Queen Mary's Hospital:- Integrated Bone Health and Falls Service- Day Hospitals- Care Home in-Reach
Main duties of the job
We are looking for an experienced and enthusiastic Band 8a AHP leader to bring knowledge and experience of working with people who are living in the community, and who has an excellent understanding of the needs of older people particularly in the area of falls/bone health management, and adults with multiple long term conditions including frailty.
We have a strong improvement culture within our services, and you will be expected to lead on quality improvement as part of wider service & pathway developments.
You must possess excellent communication and team leadership skills, working autonomously as well as collaboratively with other stakeholders to achieve service and organisational goals. Support will be provided by the therapy management team and AHP Consultant.
You will work across the Community services and liaise with health and external provider organisations in the development of services and provide clinical expertise at relevant forums.

- To work in conjunction with the AHP Therapy Consultant posts in Acute Rehabilitation and the Head of Community Therapies to address issues with peers regarding the patient pathway and quality of care
- Provides expert opinion to managers, which supports the strategic direction of community therapies and the overall Therapies service across the whole organisation utilising the principles of clinical governance, evidence-based practice and quality assurance.
- Identifies and leads clinical research, audit and data collection priorities within area of expertise to drive departmental clinical development strategies, closely working with AHP Consultants.
- To deputise in the absence of the Consultant AHP or Head of Therapies as required.

- AHP degree & HCPC registration
- Evidence of Continued Professional Development including attendance at recent post graduate courses relevant to the clinical field, demonstrating personal commitment to advanced clinical and professional development
- Leadership & Management Training

- Recognised post-graduate qualification (e.g. MSc, PG Dip)
- Membership of relevant Special Interest Group within the Professional Body (CSP/COT)
Experience
Essential
- Has relevant NHS experience which can include undergraduate experience
- Extensive post graduate experience including areas in acute/community care at Band 7 level (medical and frail older patients) covering rehabilitation, Emergency Department, Fall and Frailty or equivalent experience in a Primary Care rapid response team
- Broad range of experience at band 5, band 6 and band 7 levels of a wide range of approaches to the management of rehabilitation patients. Highly specialist knowledge and experience in acute/community medicine or older people demonstrated by advanced clinical reasoning
- Experience of undertaking clinical audit and/or research and demonstrating Experience in interpreting and implementing evidence based practice
- Experience of managing and supervising therapy staff including the full spectrum of daily line management, clinical supervision, management of attendance (annual leave, sickness etc) performance management and organizing cover
- Experience of teaching and training in the work place
- Experience of multidisciplinary team working
