Job summary
Key responsibilities will include supporting the in-patient Diabetes team meeting the increasing demands of the diabetes service across the Trust.
The post holder will provide high quality patient care through dedicated administrative and clerical support as part of the multi-disciplinary team. You will be the point of contact for collecting, collating, and presenting the data related to the team's performance in direct and related key outcome measures.
The post holder will also support the team with administrative tasks relating to the inpatient pathway of care. You will be accessible, customer focused, visible and have the skills to prioritise tasks with discretion to organise a varied daily workload.
The post holder will co-ordinate and monitor diabetes educational support across the Trust.
There is opportunity to develop this post in collaboration with the Inpatient Diabetes Team.
Main duties of the job
- Collate the activity data for the team and general data input of patients with diabetes in the Trust by liaising with the information team.
- Assist the team with the booking of ward discharge patients to appropriate clinics.
- Taking minutes at relevant departmental meetings.
- Provide updates on diabetes clinical performance, inpatient activity as managed through agreed KPIs.
- Ensure that essential communications and actions that require administration are followed up and escalated appropriately.
- To work as part of a multi-disciplinary team maintaining efficient systems and processes relating to patient attendances and admissions. This includes maintaining accurate computerised and manual records.
- To accurately record patient activity on the registration systems, including inpatient, day case and outpatient clinics and keep all relevant clinical systems up to date.
- Ensure how to liaise with IT support over the iCLIP processes.
- To liaise with Clinical Coding Department to ensure that records are accurately coded following all admissions.
- To arrange for patient feedback on the service provided and feedback information to the team.
- Promptly answer telephone enquiries, taking and relaying messages in a polite and helpful manner, taking appropriate actions and recording on iCLIP.
