Job summary

We are recruiting experienced rehabilitation practitioners to join our integrated falls and bone health team. Based at St John's Therapy Centre in Battersea, the successful applicant will work within an established multidisciplinary team (Occupational Therapy, Physiotherapy, Dietitian and SALT). You will work alongside clinicians to deliver high quality intervention and rehabilitation to patients within care homes across Wandsworth.

The post would be an exciting opportunity to move into a health team that works in partnership with many other community providers. The service has been running for over a year now, has expanded in its remit, and funding has been extended until July 2024

Rehabilitation includes activities such as prescribing and monitoring exercise programmes that enable patients to become independent in their chosen daily activities. The successful applicant would see patients on an individual basis and will lead or co-facilitate group sessions to support care home staff to deliver highly effective exercise and wellbeing sessions. You will also take an active role in the contracture prevention project in the team

Access to your own transport (bike or car) is desirable as you will be working across a number of care homes.

Main duties of the job

You will be working as an exercise facilitator as part of a multidisciplinary in-reach care home team. Improving clinical input into care homes and tailoring care around the diverse needs of individual residents can improve quality of care and quality of life for people and reduce unnecessary admission.

We are looking for a highly motivated, flexible candidate to join a new team providing community in-reach programme for older adults in care home settings. The team is part of a wider Older people's service alongside day hospital and community therapy team based at St John's Therapy Centre.

The core team includes Physiotherapy, Occupational Therapy, Dietitian, Clinical Nurse Specialist, Doctors, Speech and Language Therapist,.
